No Two Dermatology Clinics Are The Same...

...but many deal with similar issues. We bring our experience from working with many other dermatology groups to help you navigate the key questions for your practice.

How much of my practice should be cash-pay?

Botox, fillers, lasers, and other aesthetics can be very profitable — no insurance hassle, high margins, repeat visits. But the more you lean into cash-pay, the more you need clean separation between medical and cosmetic billing. Getting the balance right means maximizing revenue without creating compliance risk on the medical side.

Do I need MDs or can I use NPs and PAs?

NPs and PAs can handle a lot of dermatology — acne, eczema, routine skin checks, biopsies, and cryo. That frees your dermatologists for Mohs and complex cases. However, Mid-level billing is typically 85% of what MDs can get and it can turn patients off for more complex procedures. Supervision requirements vary by state, so be sure to check yours to ensure you are compliant.

Should I do my own Mohs?

Mohs surgery is one of the highest-reimbursing procedures in dermatology, and keeping it in-house means you capture the full value instead of referring out. But it requires a fellowship-trained surgeon, a histotech, lab space, and a billing workflow that handles the multi-stage coding correctly. For practices with enough skin cancer volume, it can be a significant revenue driver. We recommend this to practices with an experienced MD since it can be a tricky piece to stand up.

Frequently Asked Questions

How does Atlas handle cosmetic versus medical billing for dermatology? +

We review documentation for every encounter to determine whether services meet medical necessity criteria or are cosmetic. For procedures in the gray area — like removal of lesions that are both symptomatic and cosmetic — we ensure your documentation supports the medical indication and apply appropriate diagnosis codes. We also help your practice set up clean workflows for collecting patient responsibility on non-covered services.

How long does the transition take? +

Most practices are fully transitioned within 30 days. We run in parallel with your current billing operation during the overlap period so there is zero disruption to your cash flow and no claims fall through the cracks.

Do you provide a dedicated billing manager? +

Yes. Every Atlas client gets a dedicated billing manager with a direct phone number you can call or text anytime. Your billing manager knows your payer mix, your fee schedules, and your practice inside and out.

Are there long-term contracts? +

No. We work on month-to-month agreements because we believe you should stay with us because of results, not because of a contract. Most of our clients have been with us for years.
